Statistically, Those Who Don’t Go to a Subacute rehabilitation center after surgery have more difficulty recovering and often return to the hospital within 30 days. The comprehensive subacute care offered at our centers are designed for patients of all ages who require medical monitoring or rehabilitation services to build strength and endurance. Rehabilitation services are offered seven days a week. Patients receive two to three hours of intensive therapy each day while staying in a hotel-like environment as opposed to 45-60 minutes usually provided at outpatient centers.

Whether patients are admitted for sub acute rehabilitation, post surgery care, or long-term care, our nursing department is highly qualified to care for all our patients needs. Under the auspices of our medical director our specialized units allow us to care for our patients in a unique environment best suited for their individual needs. Our facility encourages family members to be actively involved with the progress and care of their loved ones.

Whether you or a loved one needs long-term nursing care or a temporary place to rehabilitate after an illness or injury, the Jefferson Health Care Center offers the highest quality care, in a modern, yet home-like setting. Our 111-bed long-term facility features modern private and semi-private rooms for residents, a spacious dining room and a day room that opens onto a lush courtyard. The Center provides a wide range of activities for every interest—from pet therapy to seasonal activities—that encourage independence, social interaction, and comfort.
